What is a care coordinator?

Navigating care systems is hard. The goal of our care coordination is to lessen the stress of making, getting to, and navigating appointments.

You can think of a care coordinator as a new member of your support system. This person can assist you in organizing all of the things that go into taking care of yourself both physically and mentally with your current providers and if a new need arises, they can help you make connections to get it taken care of.

What to expect from care coordination

Care Coordinators do not go into the community to serve clients, all services are done via telehealth, phone, or in-person appointments. A Care Coordinator doesn’t go with clients to meetings or assist in-person at appointments. Care coordination is proven to help you be more satisfied with your care, have better health outcomes, require less emergency services, reduce gaps in your care, and provide reminders for appointments.
