Community Support
 Is it getting harder to take care of yourself?
Lutheran Social Services of Illinois offers services that can help you live independently in the community and attain a better quality of life. LSSI offers adult case management, Community Support Teams, and Assertive Community Treatment depending on your level of need. All of these services come to you in your home or assist you in the community. Â
How will you know which level of care is right for you? Our clinical staff will work with you to determine which level of care will work best for you.Â
Team members will provide services based on LSSI’s Safety Protocols.

Case ManagementÂ
Helps adults with mental health issues get the support they need by finding doctors, housing, jobs, and/or other services and helping them get to those places. Case managers can act as advocates and support as you go about your life. Â
Community Support TeamsÂ
A team of therapists, social workers, case managers, nurses, psychiatrists who help with things like therapy, medications, finding housing, attending appointments, learning life skills, and staying healthy.  Â
Assertive Community TreatmentÂ
This team is similar to a Community Support Team, but includes addiction treatment counselors disorders and offers a higher level of support than a CST team. ACT is for people with more severe mental illness and who may have addiction They help you reduce the need for the hospital and give 24/7 support.Â
Clients are able to access the team 24 hours a day, 7-days a week. If they are experiencing a mental health crisis, a team member will travel to the client’s location to provide support.Â
Teams offer help in the community through:
- Therapy
- Accompaniment to appointments (medical, dental, therapy, etc.)
- Medication monitoring
- Teaching of coping skills and management of their symptoms
- Accessing transportation
- Accessing community services (food, clothing, utility assistance, etc.)
- Assistance signing up for benefits (healthcare, disability, social security)
- Housing
- Learning life skills (budgeting, grocery shopping, hygiene, laundry, etc.)
- Establishing routines and organizing
- Crisis planning
- Social Security Representative Payee
- Linkage to vocational programs
You may qualify for community support team services if:
- Have a mental health condition like schizophrenia, bipolar disorder, or something similar
- Have anxiety or depression that makes life hard
- Have been in the hospital a lot
- Are dealing with addiction
- Are having trouble living on your own
- Are homeless or were recently
- Have been arrested or in jail often
LSSI’s CST and ACT work closely with the client’s support system including physicians, family/loved ones, therapists, pharmacists. If the client doesn’t have a support system, then the team will work with the client to create one.Â
