The Homeless Outreach and Meaningful Engagement (H.O.M.E.) team works on the north and northwest sides of Chicago with those who are unhoused or facing housing instability.

How does the H.O.M.E. team work?
- This mobile team meets people where they are at in the communities we serve.
- The team assess the individual’s needs and helps coordinate their care.
- The team also connects clients to other external resources from which they may benefit.
Who does the H.O.M.E. team work with?
If you or a loved one is unhoused with no support you may qualify to work with the H.O.M.E. team to get connected to shelter and resources. You also may qualify if you are currently housed but are “housing unstable.”
What does it mean to be “housing unstable?”
You don’t have to be unhoused right now to seek help from H.O.M.E. If you know you are at risk of losing housing in the near future and have nowhere to turn for support, you may also qualify for services. H.O.M.E. works with individuals and families who are within 14 days of losing their housing, including housing they own, rent, are sharing with others, or are living in without paying rent. You may also qualify if you are about to leave systems of care such as foster care or a mental health facility.
While a mental health diagnosis is not a requirement for admission to the program, a diagnosis may be necessary for linkages to other programs for additional services. H.O.M.E. will make every effort to link clients to programs for which they are eligible, both internal and external.
