Case Management Services
For residents in northern Illinois (Carroll, Lee, Ogle and Whiteside counties), LSSI’s case managers assist seniors with obtaining services and/or devices that will help them remain in their homes as long as possible. LSSI first identifies and arranges for needed services, and then provides follow-up and re-evaluations. Typically, the services LSSI coordinates include:
- Emergency home response systems;
- Assistive technologies (devices or services that could improve or preserve the mobility and autonomy of a client);
- Respite care and home care;
- Home modifications.
Before arranging for services, LSSI evaluates whether a senior’s situation meets the state’s requirements to receive benefits. If a product or service is approved, the case manager maintains contact with the individual to ensure that services are being provided effectively or that the product is safely installed.
Funding for these services is made available through grants from the Illinois Department on Aging and the Northwestern Illinois Area Agency on Aging.
