Case Management Services for Seniors
Lutheran Social Services of Illinois (LSSI) offers case management service for seniors in northern Illinois.
LSSI provides a comprehensive care coordination assessment, arranges for needed services, and provides follow-up and re-evaluations. This program works to help older adults remain in their homes as long as possible.
LSSI also provides assessments for emergency home response systems, assistive technologies – which include devices or services that could improve or preserve the mobility and autonomy of a client – and other services – including respite care and home modifications – that might benefit seniors. The assessments include a review of the senior’s situation to see if it meets the state’s requirements to receive the benefits. If a product or service is approved, the case manager maintains contact with the individual to ensure that services are being provided effectively or that the product is safely installed.
Funding for these services is made available through grants from the Illinois Department on Aging and the Northwestern Illinois Area Agency on Aging.
