Psychosocial Rehabilitation and Case Management
(English and Spanish)

Lutheran Social Services of Illinois (LSSI) provides mental health support services for individuals with severe and persistent mental illness through its Journeys program.

Journeys is located at the Portage-Cragin Counseling Center on the northwest side of Chicago. Services are offered for both English- and Spanish-speaking individuals.

Services offered through Journeys include:

  • Psychosocial Rehabilitation Service: Journeys is designed to provide adults age 18 and older a safe place to learn, practice and integrate life skills to be used in their living, learning, social and working environments.
  • Community Support Services: are also offered for individuals who need supportive services in the community.

These services are certified and funded by the Illinois Department of Human Services’ Department of Mental Health. LSSI accepts Medicaid and offers a sliding scale of fees based on income and family size.

Case Management

Case Management is offered at two LSSI offices on the northwest side of Chicago and provides short- and long-term case management services, including supportive services around housing, funding, mental health treatment and substance abuse treatment for persons with mental illness.

Ayuda Latina (Case Management Services for Latinos)

Lutheran Social Services of Illinois (LSSI) provides case management services to adults in the Latino community on the northwest side of Chicago at its Portage-Cragin Counseling Center. The Ayuda Latina Case Management Program serves adults who are either monolingual Spanish-speaking or have limited ability to speak English.

Ayuda Latina provides case management and community support services to mentally ill Latino adults to help them successfully integrate into the community and to promote their self-sufficiency. The clients receive help with entitlements, housing, immigration issues, advocacy and community integration.

The program addresses language barriers that impede their recovery and also focuses on counseling and education for the client and family.

These services are certified and funded by the Department of Human Services’ Department of Mental Health. Ayuda Latina accepts Medicaid, and offers a sliding scale of fees based on income and family size, if the client is capable of paying for services.
