Intouch Home Care provides non-medical assistance for older adults in their homes. Trained home care assistants – CNAs – help seniors with tasks such as housekeeping, laundering clothes, meal planning, minor home maintenance and grocery shopping. They also assist with personal care, provide medication reminders and transportation to appointments, and can also provide respite care in some locations. LSSI’s Home Care Services are available in Cook, DuPage, Jo Daviess, Stephenson, Winnebago, DeKalb, Fulton, Marshall, Peoria, Stark, Tazewell and Woodford Counties.

Older Adult Care Coordination is offered for residents of Peoria, Fulton, Marshall, Stark, Tazewell and Woodford Counties. In this program, a Registered Nurse (RN) works with individuals to develop a comprehensive care plan that connects older adults with appropriate community-based programs. The RN also does a follow-up check, gives the senior’s family periodic updates and regularly reviews the client’s needs, revising the care program if necessary.
